IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We will also ask to see your driver's license or other identifying documents.

To open a new account, follow the instructions below:

1. Fill out a New Account Application and mail it, along with a copy of your driver's license and a check for a minimum of $100, to:

AIBM
6385 Old Shady Oak Rd, Ste 110
Eden Prairie, MN 55344

A banker will then call you to complete the account-opening process.

Once your account has been established:

2. Complete the Direct Deposit Change Form. You will need to complete a form for each direct deposit you currently have established, including payroll, automatic deposits or dividends.

3. Complete the Automatic Withdrawal Change Form. You will also need to complete a form for any established automatic withdrawals including payments for services/items such as:
